外国人工作许可证注销业务申办指引
《外国人来华工作许可证》有效期届满未延续的，自动注销。依法被撤销、撤回的，以及许可证件依法被吊销的，由决定机构注销。申请人死亡或者丧失行为能力或者提前终止合同、解除聘用关系的，用人单位应当于事项发生之日起10个工作日内向决定机构申请注销。
一、办理程序
网上申请→ 网上预审→ 受理→ 审查→ 决定

二、材料清单及要求
所需材料如下：
1.《外国人来华工作许可注销申请表》（系统中填写好申请人信息,在线导出申请表,打印后需申请人签名，加盖单位公章）；
2.聘用关系解除、合同终止或其他与注销原因相关的证明材料（申请人签字并加盖单位公章，详见外国人来华工作许可服务手册）；
3.《外国人工作许可证》；
4.如不能提供工作许可证，请登载遗失公告并截图上传。如不能提供申请人签名，请上传公司申请书。
5.个人所得税纳税清单（用工资条件申请才需要提供此项材料）

Guide for Cancellation of Foreigner’s Work Permit of FWP
The Foreigner’s Work Permit shall be canceled automatically if not extended before the date of expiration. The approving agency will cancel those deactivated, withdrawn or revoked according to regulations. In case of death/incapacity of the applicant, or termination of contract/employment, the employer shall apply for canceling the FWP with the deciding agency within 10 working days after the foresaid events.

2. Checklist and Requirements
The documents required are listed as follows:
2.1 Application Form for Cancellation of Foreigner’s Work Permit (the applicant should fill in his/her personal information in the system, download the completed application form, print it, sign it and stamp it with the official seal of the employer).
2.2 Documents to prove the termination of employment relationship, termination of contract or other reasons for cancellation (signed by the applicant and stamped with the official seal of the employer,(refer to ‘Handbook for foreigner’s Work permit in China’ for details).
2.3 Foreigner’s Work Permit.
2.4 If the work permit cannot be provided, please post a notice of loss and upload a screenshot of it. If the applicant’s signature cannot be provided, please upload the company application form.
2.5 Personal Tax Records of the PRC (required only when applying with income conditions).
